The text examines the fragility of Molise’s healthcare system, shaped by depopulation, population ageing, geographic isolation and infrastructural weaknesses that hinder access to care, especially in inland areas. These structural issues are compounded by a healthcare receivership that has persisted for over fifteen years, limiting regional planning capacity and negatively affecting compliance with Essential Levels of Care and patient mobility. The analysis also highlights weaknesses in community and primary care, including workforce shortages and difficulties in supporting highly vulnerable populations.
