The article examines the organization and evolution of the regional health system in the Aosta Valley, focusing on the constitutional principle of equality in access to healthcare services. Despite its financial autonomy and exemption from national spending constraints, the regional system suffers from structural weaknesses, such as hospital centralization, limited territorial healthcare, and staff shortages. These issues have led to long waiting lists, increasing reliance on private healthcare, and a growing trend of healthcare mobility outside the region. The paper also highlights recent reform initiatives, including the strengthening of community-based healthcare and telemedicine, while questioning their effectiveness in addressing the unique geographical and demographic challenges of the region. Ultimately, the analysis emphasizes the need for an integrated model capable of ensuring universal and equal healthcare access, consistent with national and constitutional requirements.
