The article examines the right to health protection in the Sicilian Region, focusing on the often problematic balance between the public healthcare system and the accredited private sector. The first section analyzes the financing mechanisms of Sicily’s healthcare system and their impact on ensuring the Essential Levels of Care (LEA), in light of the regional Recovery Plan and relevant health and financial data. The discussion then turns to the relationship between the public and private sectors, exploring the current model of cooperation, the allocation of budgets, the issue of waiting lists, and the phenomenon of «extrabudget», all of which highlight persistent systemic weaknesses. The final section addresses territorial healthcare services, emphasizing organizational inefficiencies and the reform perspectives introduced by the National Recovery and Resilience Plan (PNRR). The analysis reveals a complex picture, where the strengths of some virtuous practices coexist with the weaknesses of a fragile and uneven system, still in need of structural interventions to fully guarantee the right to health for Sicilian citizens.
