The article begins with a brief description of the main ongoing processes that are causing the emergence of a new demand for health, characterized by complexity related to multi-chronic conditions, non-self-sufficiency and disabilities, and social vulnerabilities and marginalization. It presents systemic and structural integration, with its five fundamental dimensions, as an essential condition to generate an appropriate, effective, and lasting response to the new forms in which the health needs of individuals, families, and communities manifest. Major ongoing reforms in territorial healthcare, social and health care, and social assistance are reviewed to highlight the main drivers of innovation and transformation of the current organizational frameworks of health and social services. The possibility of building true integrated local systems based on a structured relationship between the health district and the social territorial area is briefly discussed, since shared territorial relevance constitutes the real basis for integration and transformation of current divisional services. Finally, some concluding considerations address the relationship between a new capacity to respond to complex health demands and the founding constitutional orientation of Essential Levels both in health and social fields.
