Making medical decisions on behalf of patients with intellectual and developmental disabilities amounts to a crucial challenge with clinical, legal, and ethical implications. Chief among these concerns is the adequate inclusion and protection of these patients’ values and preferences, given the total or partial lack of the cognitive standards traditionally required to exercise autonomous agency. In this article, I reconstruct some of the main approaches, guidelines, and frameworks in the literature on assisted medical decision-making, focusing on the well-established and widely adopted model of supported decision-making. However, within this framework, the alternative approaches on the table often struggle to distinguish themselves from one another, particularly in terms of their clinical implementation. I suggest here that Rational Decision Theory tools can provide useful conceptual lensens for examining and modelling the different accounts of assisted medical decision-making, as well as fostering the debate through the integration of specific shared decision-making approaches and techniques, from team reasoning to solution thinking.
